We offer preventative health care services and chronic health care management typical of a primary care office. A comprehensive history and physical will be performed annually and is included in the monthly fee, in addition to any future sick or routine office visit exams. Labs, injections, other testing, home visits, etc. will incur an additional fee and discussed prior to performing or ordering services.

We are not a weight management or pain management clinic. Opioids will not be prescribed on a routine basis, nor weight loss medications without other comorbid conditions.

We offer monthly membership rates based on age and length of contract, ranging from $60-80 per month before any discounts. After an initial conversation, we’ll provide a transparent quote with no hidden costs.

Collaborative, honest, and straightforward. We're here to guide the process of optimizing your health. If for any reason the patient or doctor decides to terminate the contract, the contract will end in 30 days to allow sufficient time for transfer of care to another provider.
